JotForm Examples

With JotForm, you will be able to create different types of online/web forms. The examples are:

Contact form

  • A contact form is typically an online form that you place on your website or landing page.
  • You will let the visitor/user enter his/her name, email address, and message through your online contact form.
  • After the visitor/user submits the form, an auto reply email will be sent to him/her, notifying him/her that you have received his/her message.

Payment form

  • You can place a payment form on your website to receive payments from your website visitors.

Survey form

  • You can add a survey form on your web page, and ask your customers to answer the survey questions.
  • A survey form (with specific questions) is a method to get feedback from customers, to see whether they are satisfied with your products and/or services. After reviewing the customer survey feedback, you may be able to figure out the solutions to improve your products and/or services.

Create Online Contact Form with JotForm

A typical contact form on your website or landing page should have at least 3 entry fields:

  • Name field
  • Email address field
  • Message text area field

Create new form (JotForm)

Click Create Form.

Create form (JotForm with 3 options)

You can create your new form in one of the three ways:

  • Start a new form from scratch.
  • Use a form template.
  • Import an existing form that you have previously created.

In this example, we are going to start a new form.

You will have to choose from one of the form layouts.

  • Classic form: This is a single-page form. You will show all questions on one page.
Card form: This is a multi-page form. Usually your form will have multiple questions, and you will show one question per page.

We will choose Classic form, in our example.

Add form element (JotForm)

You will click the + Add Form Element button.

Later you will have to drag the required elements and drop them onto your form.

Contact form (JotForm sample)

You will create your first JotForm contact form by adding elements including:

  • A sub title
  • A pair of entry fields for first name and last name
  • An entry field for email address
  • A multi-line text entry area, which is a long text
  • The submit button

Set Up Email Notification

JotForm has two types of email alerts available. One of them is the email notification.

Immediately after a website visitor has submitted an online form on your website or landing page, an email notification is automatically sent to your email address.

JotForm Menu Settings

Click the SETTINGS link at the top in your JotForm form builder account.

Sub-menu under Settings - Choose Emails

This sub-menu will pop up as a sidebar on the left.

Now choose EMAILS (Send emails after submissions).

New Email Notification or Auto Responder

Choose NOTIFICATION 1.

This will open the email notification template.

Notification Email Content (JotForm)

Under the Email tab, enter a subject for your notification email.

In the email content, the template as it is shown is going to send you the information of the person who submits the contact form. The information includes:

  • The name of the person
  • The email of the person
  • The question (or message) submitted by the person

Enter the content of your notification email, if you need additional content.

Notification Email Recipients (JotForm)

Under the Recipients tab, you will fill in the information for these fields:

  • Sender Name: This is the name of the person who submits the contact form.
  • Reply-to Email: This is the email address of the person who submits the contact form.
  • Recipient Email: This is your email address, because you should receive this notification email.

Set Up Auto Responder

One of the two email alerts that is available on JotForm is the auto responder.

The auto responder (i.e. an automated email) is immediately sent to the website visitor who has successfully submitted the online form on your website, or landing page.

Auto Responder Email Content (JotForm)

Under the Email tab, you will enter the subject for the auto responder email.

You must enter the content of the auto responder email in Email Content.

Auto Responder Email Recipients (JotForm)

Under the Recipient tab, you will enter the following information:

  • Sender Name: This is the name of the person who submits the contact form.
  • Reply-to Email: This is your email address. The best practice is to use one of your domain name email addresses. This makes your auto responder email looks serious and professional.
  • Recipient Email: This is the email of the person who submits the contact form.

Embed JotForm onto WordPress Sites

You will have to place your contact form on to your website (or one of your web pages). In this example, we assume your website is a WordPress website where you have your own unique domain name with a paid web hosting.

Publish button - JotForm form editor

While your contact form is opened in the Form Builder, click the Publish tab.

JotForm Embed option

Select the Embed option on the left side navigation, where you will be presented with multiple embed options for web pages.

JotForm Embed copy & paste code snippet

Copy this code snippet.

Now log in to the admin area of your WordPress website’s admin area.

Create a new page or a new post. Paste the Jotform code snippet of your contact form onto the editing area of your WordPress page (or post).

Publish (or update) your page (or post).

Now the contact form that you have created with Jotform is live on your WordPress site.
